Overview
\n\u201cQuestionnaire\u201d function allows you to know the status and opinions of the students and reflect their voices in the class. The results of Questionnaire can be downloaded in batches.<\/h3>\n

The following describes the steps to create a \u201cQuestionnaire.\u201d<\/p>\n

If you want to check the level of students\u2019 understanding in class in real time, \u201cWasepochi\u201d is useful.<\/p>\n

If a response includes any private\/personal information as text answer (free description field), it becomes privacy issue, i.e., it can be viewed by other students.
\nIn order to refrain from such privacy issue, please set “Students can view ALL responses ” to “Never” in Response options.<\/dd>\n<\/dl>\n

    1. Start course editing mode.\n

    2. Click [Add an activity or resource].

    3. Choose [Questionnaire] and click [Add].
      \n\"\"<\/li>\n
    4. Enter \u201cName\u201d and \u201cDescription\u201d of the questionnaire.
      \n      \"\"<\/a><\/li>\n
    5. You can set the timing for the questionnaire.
      \n      \"\"<\/a><\/li>\n

      Set Response option.<\/div>\n
      With \u201cType,\u201d set how many times users can respond, and with \u201cRespondent Type,\u201d set whether to display users\u2019 full name or not. You can also set whether students can see all responses and which timing they can see them if allowed to see them with \u201cStudent can view ALL responses.<\/div>\n

      \u30fbIf you set “Type” to “respond many”, then the student’s second and subsequent responses will be treated as new responses (and will not overwrite the first).

      Click [Save and return to course]. Now the questionnaire is added to the course.<\/div>\n
      Click the questionnaire to open.<\/div>\n

    8. Create questions. Click [Add questions].

    9. Choose question type to add and click on [Add selected question type]. See 12-01. Questionnaire for description of each question type.

    10. Set question. This is the setting screen for \u201cYes\/No question.\u201d
      \n\"\"<\/li>\n
    11. Click [Save changes]. Now the question is added to \u201cManage questions.\u201d To add more questions, follow the same procedure.

    12. Click on [Preview] to check the created questionnaire.
      \n\"\"<\/li>\n
    13. \u00a0questionnaire which has been added at least one question will be displayed as shown below when you open it. If you want to continue adding or editing questions, display the menu from [Questions] icon on the right.
