[Vaccinations] What identity verification documents are acceptable?

Bring a document such as a student ID card, faculty/staff ID card, Individual Number Card, driver’s license (driving history certificate), health insurance card, latter-stage elderly healthcare card, long-term care insurance card, passport, or pension booklet. Please note carefully: if you have forgotten your identity verification documents, you will not be able to get vaccinated.
