Please bring the following on your day of vaccination:
1) Covid-19 booster shot (3rd dose) vaccination voucher (original, no copies allowed)
2) Pre-Vaccination Screening Questionnaire (You might have your vaccination voucher printed on it)
3) Proof of vaccination (1st and 2nd dose)
*You need to bring this if the information of your 1st and 2nd doses is not printed in your Covid-19 booster shot (3rd dose) vaccination voucher
4) Proof of identification (original)
